摘要:

目的 应用CT脑灌注成像检查方法,定性、定量分析外伤性病人去骨瓣减压术后脑血流动力学的变化,探讨颅骨缺损综合征发生的机制。
方法 选取16例头部外伤去骨瓣减压术后病人,在去骨瓣减压术后2~18个月行CT脑灌注成像检查,分别测量并比较患侧、对侧的大脑基底核、丘脑、皮质区、脑白质的脑血容量(CBV)、脑血流量(CBF)、平均通过时间(MTT)、达峰时间(TTP)。
结果 脑外伤去骨瓣减压术后病人患侧基底核、皮质区、脑白质CBV、CBF均较对侧减低,差异有统计学意义(z=2.194~2.455,P<0.05)。而双侧丘脑CBV、CBF、MTT、TTP比较差异无统计学意义(P>0.05)。
结论 脑外伤病人去骨瓣减压术后患侧脑血流灌注减低。



 

Abstract:
Objective  To investigate the quantitative and qualitative changes in cerebral hemodynamics after post-traumatic decompressive craniectomy using CT cerebral perfusion imaging, as well as the pathogenesis of skull defect syndrome.
Methods  A total of 16 patients who underwent traumatic decompressive craniectomy were enrolled. CT cerebral perfusion imaging was performed at 2-18 months after decompressive craniectomy. Cerebral blood volume (CBV), cerebral blood flow (CBF), mean transit time (MTT), and time to peak (TTP) of the basal ganglia, thalamus, cortex, and white matter at the affected side and the contralateral side were measured.
Results  In the patients who underwent post-traumatic decompressive craniectomy, the basal ganglia, cortex, and white matter at the affected side had significantly reduced CBV and CBF compared with those at the contralateral side (z=2.194-2.455,P<0.05). There were no significant differences in CBV, CBF, MTT, and TTP between both thalami (P>0.05).
Conclusion  Patients have reduced cerebral blood flow perfusion at the affected side after post-traumatic decompressive craniectomy.
