摘要:
目的 探讨单体素磁共振波谱(1H-MRS)在常规磁共振检查未发现病灶的颞叶癫痫诊断中的应用。
方法 对临床诊断为颞叶癫痫病人33例及年龄、性别与之相匹配的12例正常人行海马区域1H-MRS检查,检测两组N-乙酰天门冬氨酸(NAA)/肌酸(Cr)、胆碱(Cho)/Cr值,并计算NAA/(Cr+Cho)值。
结果 癫痫病人患侧、对侧及对照组海马区的NAA/(Cr+Cho)值、NAA/Cr值依次升高,差异有统计学意义(F=15.655、13.033,P<0.01)。
结论 1H-MRS通过显示NAA/Cr、NAA/(Cr+Cho)值的变化,在颞叶癫痫诊断中有一定价值。
Abstract:
Objective  To investigate the role and value of single-voxel magnetic resonance spectroscopy (1H-MRS) in diagnosing patients with temporal lobe epilepsy without lesions in conventional magnetic resonance imaging (MRI).
Methods  A total of 33 patients with temporal lobe epilepsy and 12 age- and sex-matched healthy subjects were enrolled in the study. 1H-MRS was performed in the ipsilateral and contralateral epileptogenic zones of patients and the hippocampus region of healthy subjects. The ratios of N-acetylaspartate (NAA)/creatine (Cr), choline (Cho)/Cr, and NAA/(Cr+Cho) were determined.
Results  The ratios of NAA/(Cr+Cho) and NAA/Cr in the ipsilateral epileptogenic zone of patients were lowest, followed by the contralateral epileptogenic zone of patients, and the hippocampus region of healthy subjects, and the differences were statistically significant (F=15.655,13.033;P<0.01).
Conclusion  1H-MRS has a certain value in the diagnosis of temporal lobe epilepsy by displaying the changes in NAA/Cr and NAA/(Cr+Cho).
