摘要:
目的 观察改良强制性运动疗法(mCIMT)对早期脑卒中偏瘫病人上肢运动功能的影响。
方法 符合mCIMT适应证的脑卒中病人57例,随机分成2组。观察组29例,采用mCIMT对其上肢功能进行强化训练;对照组28例,给予常规康复干预,两组均持续治疗8周。治疗前后分别采用Fugl-Meyer运动评价法(FMA)和Wolf运动功能测试量表(WMFT)对两组病人上肢运动功能进行评定。
结果 两组治疗后FMA评分和WMFT评分均显著高于治疗前,差异有显著性(t=3.63~6.38,P<0.05);观察组治疗前后FMA评分差值、WMFT评分差值均明显高于对照组,差异有显著性(t=3.86、2.56,P<0.05)。
结论 mCIMT能进一步改善早期脑卒中偏瘫病人上肢运动能力,是促进偏瘫病人上肢运动功能恢复的有效措施。
Abstract:
Objective  To investigate the effect of modified constraint-induced movement therapy (mCIMT) on upper extremity motor function in patients in the early stage of apoplectic hemiplegia.
Methods  A total of 57 patients with cerebral apoplexy who met the indications for mCIMT were enrolled and randomly divided into two groups. The 29 patients in observation group were given mCIMT to enhance the motor function of the upper extremities, and the 28 patients in control group were given conventional rehabilitation intervention. Both groups were treated for 8 consecutive weeks. Fugl-Meyer Assessment (FMA) and Wolf Motor Function Test (WMFT) were used to evaluate upper extremity motor function before and after treatment.
Results Both groups had significant increases in the FMA and WMFT scores after treatment (t=3.63-6.38,P<0.05), and the observation group had significantly greater increases in the FMA and WMFT scores than the control group (t=3.86 and 2.56,P<0.05).
Conclusion  In patients in the early stage of apoplectic hemiplegia, mCIMT can further improve their upper extremity motor function, and therefore, it is an effective measure for the recovery of upper extremity motor function in patients with hemiplegia.
