摘要:
探讨2型糖尿病病人甲状腺功能状态与代谢综合征(MS)的相关性。
方法 选取2014年1月—2015年12月在我院确诊的2型糖尿病病人308例,分析年龄、性别、甲状腺功能状态与MS检出率及其组分的相关性。
结果 老年组MS检出率高于中、青年组(χ2=12.491、6.710,P<0.05);女性MS检出率高于男性(χ2=10.015,P<0.05)。临床甲状腺功能减退组、亚临床甲状腺功能减退组和甲状腺功能正常组MS检出率差异无显著性(P>0.05)。与甲状腺功能正常组比较,临床甲状腺功能减退组三酰甘油(TG)、糖化血红蛋白(HbA1c)、舒张压(DBP)升高(F=3.478~6.731,P<0.05),亚临床甲状腺功能减退组HbA1c、DBP升高(q=9.057、7.926,P<0.05)。与非MS组比较,MS组促甲状腺激素(TSH)水平显著升高(Z=-2.549,P<0.05)。Spearman相关分析显示,女性TSH与体质量指数(BMI)、收缩压(SBP)、TG呈正相关性(r=0.182~0.262,P<0.05),游离甲状腺素(FT4)与HbA1c呈正相关(r=0.209,P<0.05),游离三碘甲状腺原氨酸(FT3)与DBP呈正相关(r=0.291,P<0.05);男性TSH与BMI、TG呈正相关,与高密度脂蛋白(HDL-C)呈负相关(r=-0.211~0.197,P<0.05),FT4与总胆固醇(TC)呈负相关(r=-0.197,P<0.05),FT3与BMI、SBP、DBP呈正相关(r=0.189~0.287,P<0.05)。以肥胖/体质量超标为应变量、不同水平TSH为自变量行Logistic多元回归分析,校正年龄、性别后,TSH高组发生肥胖/体质量超标的危险性显著高于TSH中组(OR=3.598,P<0.05)。
结论 甲状腺功能降低、年龄≥60岁、女性2型糖尿病病人发生MS的危险性增加。
Abstract:
Objective  To investigate the association between thyroid function and metabolic syndrome (MS) in patients with type 2 diabetes.
Methods  A total of 308 patients diagnosed with type 2 diabetes in Qingdao University Affiliated Hospital from January 2014 to December 2015 were selected in this study. The correlations of age, sex, and thyroid function with the detection rate of MS and the components of MS were analyzed.
Results  The detection rate of MS in the elderly group was significantly higher than that in the middle-aged and youth groups (χ2=12.491 and 6.710, both P<0.05), and it was significantly higher in females and in males (χ2=10.015,P<0.05). There was no significant difference in the detection rate of MS between clinical hypothyroidism group, subclinical hypothyroidism group, and euthyroidism group (P>0.05). Compared with the euthyroidism group, the clinical hypothyroidism group had significantly increased triglyceride (TG), hemoglobin A1c (HbA1c), and diastolic blood pressure (DBP) (F=3.478-6.731,P<0.05), and the subclinical hypothyroidism group had significantly increased HbA1c and DBP (q=9.057 and 7.926,P<0.05). Compared with the non-MS group, the MS group showed a significant increase in thyroid-stimulating hormone (TSH) level (Z=-2.549,P<0.05). Spearman correlation analysis showed that in females, TSH was positively correlated with body mass index (BMI), systolic blood pressure (SBP), and TG (r=0.182-0.262,P<0.05), free thyroxine (FT4) was positively correlated with HbA1c (r=0.209,P<0.05), and free triiodothyronine (FT3) was positively correlated with DBP (r=0.291,P<0.05); in males, TSH was positively correlated with BMI and TG, but negatively correlated with high-density lipoprotein (r=-0.211 to 0.197,P<0.05), FT4 was negatively correlated with total cholesterol (r=-0.197,P<0.05), and FT3 was positively correlated with BMI, SBP, and DBP (r=0.189-0.287,P<0.05). Multivariate logistic regression analysis showed that after adjustment for age and sex, the risk of obesity/overweight was significantly higher in the high-TSH group than in the middle-TSH group (odds ratio=3.598,P<0.05).
Conclusion  In patients with type 2 diabetes, hypothyroidism, ≥60 years of age, and female sex are associated with increased risk of MS.
