摘要:
 目的 探讨男性2型糖尿病(T2DM)病人血尿酸(SUA)水平与胰岛素抵抗及肥胖的关系。
方法 收集188例男性2型糖尿病住院病人,按SUA分为尿酸正常组(A组)及高尿酸血症(HUA)组(B组),比较两组相关临床指标。
结果 与尿酸正常组比较,HUA组体质量指数(BMI)、胰岛素抵抗指数(HOMA-IR)等指标明显升高,睾酮(T)明显下降,差异有统计学意义(t=-2.29~4.97,P<0.05)。SUA与BMI、HOMA-IR等指标呈正相关(r=0.144~0.477,P<0.05),与T呈负相关(r=-0.215,P<0.05)。Logistic回归分析显示,BMI、HOMA-IR为男性2型糖尿病合并HUA的独立危险因素。利用易侕统计软件计算导致尿酸升高阈值,HOMA-IR每升高1单位尿酸升高8.2 μmol/L;BMI大于20.8 kg/m2时,BMI每增加1 kg/m2尿酸升高11.3 μmol/L。
结论 男性T2DM病人HUA的发生可能与胰岛素抵抗及肥胖相关。
Abstract:
 Objective To investigate the association of serum uric acid (SUA) with insulin resistance and obesity in male patients with type 2 diabetes mellitus (T2DM).
Methods A total of 188 hospitalized male patients with T2DM were enrolled and divided into normal SUA group (group A) and hyperuricemia (HUA) group (group B) according to their SUA levels, and relevant clinical indices were compared between the two groups.
Results Compared with group A, group B had significantly higher body mass index (BMI) and insulin resistance index (HOMA-IR) and significantly a lower testosterone level (t=-2.29 to 4.97,P<0.05). The SUA level was positively correlated with BMI and HOMA-IR (r=0.144-0.477,P<0.05), but negatively correlated with testosterone level (r=-0.215,P<0.05). Logistic regression analysis indicated that BMI and HOMA-IR were indepen-
dent risk factors for HUA in male patients with T2DM. Empower Stats was used to find the thresholds of HOMA-IR and BMI that induced high uric acid levels. The results showed that 1 unit increase in HOMA-IR resulted in an increase in SUA level by 8.2 μmol/L; when BMI was higher than 20.8 kg/m2, an BMI increase of 1 kg/m2 resulted in an increase in SUA level by 11.3 μmol/L.
Conclusion The risk of HUA in male patients with T2DM is associated with insulin resistance and obesity.
