摘要:
 目的 探讨经核因子kappa B(NF-κB)通路调节骨桥蛋白(OPN)表达对人膝关节滑膜炎的影响。
方法 收集20例原发性膝关节骨关节炎(OA)病人滑膜标本(OA组),以因膝关节内骨折行急症手术、半月板或韧带损伤3 d内手术病人滑膜标本15例作为对照组。采用免疫组化法观察两组膝关节滑膜中OPN及NF-κB通路相关蛋白的表达;体外分离培养获取成纤维滑膜细胞,采用Western blot方法检测膝关节成纤维滑膜细胞中OPN及NF-κB通路中p65、磷酸化p65(p-p65)、IκBα和磷酸化IκBα(p-IκBα)的表达。
结果 与对照组比较,OA组滑膜炎症明显。免疫组化显示,OA组滑膜组织中OPN表达明显高于对照组,NF-κB信号通路激活,差异有统计学意义(t=4.796~16.980,P<0.05)。Western blot检测显示,OA组成纤维滑膜细胞中OPN、p65、p-p65、p-IκBα表达较对照组显著增加,差异有统计学意义(t=3.351~16.980,P<0.05);而两组通路抑制蛋白IκBα表达差异无显著性(P>0.05)。
结论 OA滑膜增生与炎症可能与NF-κB信号通路激活介导的OPN的表达有关。
Abstract:
 Objective To investigate the effect of osteopontin (OPN) expression regulated via the NF-κB pathway on human knee synovitis.
Methods Synovial samples were collected from 20 patients with primary knee osteoarthritis (OA group), and the synovial samples collected from 15 patients who underwent emergency surgery due to knee intraarticular fracture or who underwent surgery within 3 d after meniscus or ligament injury were enrolled as control group. Immunohistochemistry was used to measure the expression of OPN and NF-κB pathway proteins. Fibroblast-like synoviocytes were obtained by in vitro isolation and culture, and Western blot was used to measure the expression of OPN, p65, phosphorylated p65 (p-p65), IκBα, and phosphorylated IκBα (p-IκBα) in fibroblast-like synoviocytes in the knee joint.
Results Compared with the control group, the OA group had significantly greater synovial inflammation. Immunohistochemistry showed that compared with the control group, the OA group had significantly higher expression of OPN in the synovial tissue and significantly activated NF-κB pathway (t=4.796-16.980,P<0.05). Western blot showed that compared with the control group, the OA group had significant increases in the expression of OPN, p65, p-p65, and p-IκBα (t=3.351-16.980,P<0.05), while there was no significant difference in the expression of IκBα between the two groups (P>0.05).
Conclusion Synovial hyperplasia and inflammation in OA may be associated with the expression of OPN mediated by the NF-κB signaling pathway.
