摘要:
目的 研究syndecan-1检测对重症急性胰腺炎(SAP)血管内皮功能评价。
方法 采集我院入院时SAP病人(SAP组,47例)、连续性肾脏替代治疗(CRRT)后SAP病人(CRRT组,13例)及健康对照者(对照组,9例)的血清,采用酶联免疫法检测血管内皮多糖蛋白复合物(syndecan-1)浓度,比较SAP组与对照组以及CRRT治疗前后浓度的变化。
结果 SAP病人与对照组血清syndecan-1浓度分别为(295.28±126.53)、(13.00±3.06) mg/L,差异有统计学意义(t=10.953,P<0.001)。CRRT治疗前后血清syndecan-1的浓度分别为(533.36±116.00)、(247.09±124.98) mg/L,差异有统计学意义(t=9.513,P<0.001)。
结论 血清syndecan-1可以作为一项检测和监测SAP病人体内血管内皮功能的指标,对预测病人的预后可能有意义。临床上使用CRRT治疗可以明显减轻SAP病人血管内皮细胞结构和功能的损害。
Abstract:
Objective To investigate the value of syndecan-1 measurement in evaluating vascular endothelial function in patients with severe acute pancreatitis (SAP).
Methods A total of 47 patients with SAP on admission (SAP group),13 SAP patients who underwent continuous renal replacement therapy (CRRT) (CRRT group), and 9 healthy controls (control group) were enrolled. Serum samples were collected from all subjects, and ELISA was used to measure the concentration of syndecan-1 in the vascular endothelium. The change in the concentration of syndecan-1 after treatment was compared between groups.
Results There was a significant difference in the concentration of sydecan-1 between the SAP group and the control group ((295.28±126.53) mg/L vs (13.00±3.06) mg/L; t=10.953,P<0.001). In the CRRT group, the serum level of sydecan-1 decreased significantly from (533.36±116.00) mg/L to (247.09±124.98) mg/L (t=9.513,P<0.001).
Conclusion Serum syndecan-1 can be used as an index for evaluating and monitoring vascular endothelial function in SAP patients and has a certain value in predicting patients’ prognosis. In clinical practice, CRRT can significantly alleviate the damage in the structure and function of vascular endothelial cells in SAP patients.
